How will the Private Leaderboard be scored, and how should we submit our code and models?

The Public Leaderboard is scheduled to close on February 2nd, with the Private Leaderboard evaluation to follow. Currently, only model predictions are being submitted; when should we submit the actual models and code for the private phase? Furthermore, what is the required procedure for submitting them?
